HealthCare Pharmacy’s transition process includes the following:

• Provide a "point-of-contact" person for answering any questions that may arise
  
  
• Coordinate the gathering of all pertinent resident information, including medical records containing all the medication orders, resident pay types, and third party insurance information
  
  
• Evaluate and set-up of emergency and back-up medications and supplies
  
  
• Select the type of medication and treatment carts
  
  
• Coordinate delivery of all necessary start-up supplies and reference materials
  
  
• Schedule and provide in-services for facility education
  
  
• Assist in notifying residents' families of the pharmacy change
  
  
• Meet with families and residents, as requested
